That is car # 1; originally the second or third car # A-1. (Yes, the ARR liked to renumber and reuse numbers).
It is a former USATC Hospital car which I think was USA # 89356 built by AC&F in 1944 or 45. It came to the ARR in March 1949. It was out of service early and was scrapped in the late 1960's. It's a bit of a mystery as to what modifications were made to the interior (if any); the exterior looks like the Army hospital car. I have a note from the late John Henderson that it was used to support the health dept. car in the early 1950's along with another un-repainted USATC hospital car. Both cars were stored for a long time in Anchorage, later moved to Birchwood and both scrapped.
This car doesn't show up on many ARR rosters; I haven't seen any correspondence about it.
The attached photo is a scanned slide from my collection; photographer, date and location unknown. The car to the right is probably the un-repainted car mentioned. I think I have an unscanned negative from John Henderson of that car right before it was scrapped; I'll have to check. The cars look to have been retired in this view.
